If your surgery will be performed in the main operating rooms of the
Jules Stein Eye Institute, you will need to have an internist or
general practitioner complete a Preoperative History and Physical
Examination form. If you have a significant cardiac history and are
under the care of a cardiologist, please have your cardiologist
complete the form. It is your responsibility to see that the form is
brought with you or faxed to the pre-anesthesia office. Do not mail
these forms. The pre-anesthesia staff will review the completed form at
the time of your preoperative evaluation. If you will be having
refractive surgery or a laser procedure, it will not be necessary to
have a preoperative history and physical examination.
