UCLA has contracts with all of the major health plans and numerous medical groups. Preferred Provider Organization (PPO) members are able to receive care at the Jules Stein Eye Institute without prior authorization.

HMO & POS Plans
If you are a member of a Health Maintenance Organization (HMO), you must receive prior authorization from your insurance company or carrier before making an appointment in order to receive any insurance coverage. However, many Point-of-Service (POS) plans have an out-of-network provision that allows patients to receive care at our Institute at a higher out-of-pocket cost without an authorization.

Our Advice
It is difficult for the Institute to know what is covered by the individual managed care plans. We suggest that you contact your plan’s patient care services department to check the details of your coverage since you are responsible for payment of uncovered services.
